Nametag Replacement Requests
If you need to replace a name tag or replace the sticker on a name tag, you can use the form linked below.
Important reminders:
- You can request up to 3 replacements at a time.
- For each name tag, please make sure you select if it is the "Sticker Only" or "Full Name Tag."
- If name tag replacement requests are excessive from the same associate, HR may choose to charge the associate for a replacement.
